Consultation Services
Providence Alaska Medical Center offers a Palliative Care Consultation Service. This service consists of a multidisciplinary team that will assist physicians and nurses treating patients hospitalized with pain and other symptoms as a result of serious or terminal illness. The team will also help to clarify the patient and family’s goals of care.
Physician Referral
As with all hospital consultation services, the patient’s physician requests the assistance of the Palliative Care Service team in the care of the patient. Although patients, families, and caregivers may discuss a Palliative Care consultation with the primary physician at any time, the physician must order the consultation.
Ongoing Support
Patients seen by the Palliative Care consultation team during their hospitalization may be seen in the palliative care follow-up clinic, if further outpatient assistance with their care is required.
Transition Support
The Palliative Care consultation team, in coordination with the Providence Alaska Medical Center’s case management and social work services, will work with the patient, family and physician to assure smooth care transitions to home, and, when appropriate, to home health services, assisted living, skilled nursing homes, and to hospice.
